High Street Dental Practice aims to meet the requirements of the Data Protection Act 2018, The General Data Protection Regulation (GDPR) guidelines, and those of the Information Commission and professional bodies.

We need to keep comprehensive and accurate personal data about our patients in order to provide them with safe and appropriate dental care.

Personal Data that we hold includes:

• Your past and current medical and dental condition, your age, address, telephone number and your general medical practitioner.
• Radiographs and study models
• Information about the treatment that we have provided, or propose to provide, and it’s cost.
• Notes of conversations or incidents about your care, for which records need to be kept.
• Record of consent for treatment.
• Correspondence relating to you with other healthcare professionals.

Hard copy and computerised records are stored, reviewed and updated securely and confidentially. Confidential information is only seen by personnel who need to see it, and staff are trained on our policies and procedures to keep patient information confidential. We minimise the data that we keep, and do not retain it for longer than is necessary.

Reasons we have for processing the data:

• To maintain your contemporaneous clinical records
• To provide you with relevant dental care
• As a record of financial transactions
• To send your personal data to the General Dental Council or other authority, as required by law
• To communicate with you as and when required, including appointment reminders, treatment plans, estimates and other communications about your treatment.
• Communicating with your next of kin in an emergency
• To refer you to other healthcare professionals as necessary

Your rights with regard to your personal data include:

• Correct

• Restrict
• Transfer
• Object to processing
• Portability
• Withdraw consent
